Assisted Living For High Risk Seniors
Around-the-clock scheduled and unscheduled support can be now accessed by an eligible high-risk senior who is living independently in either an Assisted Living (AL) residence or near one in their own home. In the past, AL was building focused. Now, thanks to an updated High Risk Seniors Policy it can be delivered outside of a building and in areas where large numbers of seniors are living near a provider.
The NE LHIN is in the process of designating these areas around its 17 AL providers.
Assisted Living is different than home care as it includes both scheduled and unscheduled visits, while all home care visits are scheduled. An unscheduled visit means the senior is able to call and receive help from their AL provider within a short time period.
Eligibility:
The North East Community Care Access Centre (NE CCAC) will assess individuals to see if they meet the criteria to be considered a High Risk Senior. To contact the NE CCAC call 310-CCAC (2222). A High Risk Senior includes those who:
- Demonstrate a need for daily access to personal support and/or attendant services throughout a 24 hours period;
- Persons living in their own home or within a residential setting;
- Are able to direct their own care or have a substitute decision making or live-in caregiver to direct their care;
- Are medically stable -- their medical needs can be met by providers and their family physician;
- Poses no risk to themselves or others.
Services May Include:
- Safety and reassurance checks by phone or in person
- Personal living support
- Toileting
- Washing/bathing
- Preventative skin care
- Transferring/positing/turning
- Assistance with eating
- Reminders to take medications
- Exercising
- Meal preparation
- Bed making and laundry
- Light housekeeping
