NE LHIN Review of Rehabilitation (Rehab) and Complex Continuing Care (CCC) Systems, Fall 2011

The North East LHIN in conducting a three-month review of the current status of Rehabilitation and Complex Continuing Care programs and services in the NE LHIN. An interim summary report is planned for the end of 2011.

The review includes appraising access and flow of inpatients, hospital-based outpatients, and publicly funded community-based rehab services (i.e. CCAC and tele-rehab).

The goal of the project is to develop recommendations and an implementation plan to enhance access and patient flow to Rehab and CCC in the NE LHIN.

This project is a step towards the longer term objective of developing a comprehensive service delivery model for Rehab and CCC in Northeastern Ontario.

This initiative will support ED and ALC priorities and is in keeping with the recommendations and work of the Rehabilitation and Complex Continuing Care Expert Panel.  Phase 1 of the Rehabilitation and Complex Continuing Care Expert Panel Report was received in June 2011 by all LHINs and senior staff within the MOHLTC.
