NE LHIN Integration Strategy

Overview

In September 2008, the NE LHIN Board of Directors approved the NE LHIN Integration Strategy. The Strategy was developed to provide health service providers with information and a process to achieve success with health service integration and coordination efforts.

The NE LHIN supports integration activities that:

• enhance access to quality health care services for individuals;
• are patient-focused; and
• make the most effective and efficient use of available resources.

The two pillars of the Strategy are:

• care pathways:  integrated care per client condition; and
• shared services.
