This Environmental Scan was prepared for use by all Local Health Integration Networks (LHINs) as a key background paper for each LHIN’s third Integrated Health Service Plan (IHSP). This Scan provides an overview of a number of key characteristics of local populations, services, utilization, and health impacts.This directly contributes to an understanding of the status of many important health system attributes and allows for data comparisons between the LHINs.

The Canadian Red Cross is working closely with the North East Local Health Integration Network to implement a system navigation program aimed at identifying and addressing system barriers to better support clients and family caregivers in accessing the services available to them in the community.
 
Nov 15, 2011Enhancing the Continuum of Care
In September 2010, the ministry convened an Avoidable Hospitalization (AH) Advisory Panel and named Dr. G. Ross Baker as the chair. The Panel had a mandate to identify : system-wide AH best practice guidance, form and content of an AH improvement practices inventory, and measures and an evaluation framework for AH initiatives. Dr. Baker and his Panel have completed their work and submitted their report, Enhancing the Continuum of Care, to the ministry.

Our society now confronts a growing phenomenon – that of a burgeoning aging population of individuals living with frailty, and/or multiple co-morbidities, all of which may be confounded by the challenges of dementia. As this changing demographic becomes the reality, and the health care system’s ability to manage the diseases of aging improves, there is a pressing and urgent necessity to address the needs of this population in new ways.

On December 8th, 2010, the Ministry of Health and Long Term Care (MOHLTC) and the North East Local Health Integration Network (NE LHIN) appointed Rik Ganderton, President and Chief Executive Officer, Rouge Valley Health System as Peer Reviewer for the Sault Area Hospital.

Nov 5, 2010MOHLTC & LHIN Joint Review Framework for Early Capital Planning Stages
In November 2010, the Ministry of Health and Long-Term Care and all 14 LHINs released a new Joint Review Framework for early capital planning. Under this Framework the LHINs play a critical role in advising on, and endorsing, program service elements of all capital projects throughout the planning stages. The Ministry of Health and Long-Term Care/LHIN Capital Working Group will be launching its ‘Joint Review Framework for early Capital Planning Stages’ at HealthAchieve on Tuesday, November 9, 2010.
